Steps

  1. Check the destination country's entry rules and the airline's rules for children.
  2. Write the child's full name, birth date and passport number.
  3. Name the adult traveling with the child and their relationship.
  4. List the destinations, travel dates and where you will stay.
  5. Have the parent who is not traveling sign it with their address and phone number.
  6. Attach a copy of that parent's photo ID, and have it notarized if required or advised.
  7. Carry the original and keep a photo of it on your phone.
